Do not submit the business certificate as the document proof.) 2.Tax statement of the most recent term; any vendor who has not yet obtained the proof of tax statement for the most recent term may submit the proof of tax statement from one prior term;A newly established business who has not yet filed tax for the first term may submit the official letter confirming the business establishment issued by the business tax authorities in charge,and the document attesting to the application for the purchase of uniform invoice; 3.Proof of bidder’s credit-worthiness (i.e. having no record of a bounced check); 4.The operating license of the professional chef; 5.The certificate of professional chef issued by the trade (or industry) association recognized by the local health authorities; 6.Proof of bidder’s membership in the local culinary trade (or industry) association;
